What Is an Apicoectomy (Root-End Surgery)?

When a traditional root canal isn’t enough to resolve a persistent infection or inflammation, your endodontist may recommend an apicoectomy—a type of microsurgery that treats the problem at the tip of the root (the apex). This approach allows us to remove infected tissue and seal the root from below when retreatment through the crown isn’t possible or advisable.

Apicoectomy may be the best option in cases of:

  • Recurrent or chronic infections
  • Root fractures or blocked canals
  • Persistent cysts or inflammation
  • Previous root canal failures

How Apicoectomy Surgery Works

During the procedure, your endodontist will:

  1. Make a small incision in the gums to access the root tip
  2. Remove the infected or inflamed tissue
  3. Trim and clean the root tip
  4. Place a small root-end filling to seal the canal
  5. Close the area with a few sutures

Your body will then begin to heal, and new bone will naturally regenerate around the treated root. Any removed tissue is typically sent to a pathology lab for evaluation.

What Is Endodontic Microsurgery?

Microsurgery is a broader term that includes apicoectomies and other surgical procedures used in endodontics. It’s often recommended when previous root canal treatments have failed or when the problem can’t be identified or treated with conventional methods. In some cases, it’s also used to remove calcium deposits, repair damaged roots, or treat areas that aren’t visible in X-rays.
